Please note that the first Friday of June each year is the deadline to submit Research Ethics Board (REB) applications, requests for amendment, and renewals. Applications submitted after this date will not be reviewed until September due to operational downtime from mid-June through the end of August.
The following forms must be submitted through the St. Lawrence College Research Ethics Board (SLC-REB) submissions site
- SLC-REB Application Form (Researchers)
- SLC-REB Request for Renewal Form
- SLC-REB Request for Amendment Form
- SLC-REB Termination of Project Form
- Ontario Community College Multi-Site Research Ethics Board Application Form
- Ontario Community College Multi-Site Change Request/Amendment Form
- Ontario Community College Multi-Site Renewal/Completed Status Form
The following form must be submitted using the SLC-REB Student Course Based Research submissions site
SLC-REB Student Course-based Research (SCBR) Form (Faculty, students)
***Prior to the end of each semester, faculty with SCBR clearance for that semester must complete the SCBR Project Completion Form
