Instructions

Fill out the form below to initiate the appeal process. This form will be sent to the Student Rights & Responsibilities Officer who will contact you if you are missing any pertinent information for the appeal. Otherwise, a meeting will be scheduled with you and your faculty member.  You will receive information about Step 1 appeals via your SLC student email account.  It is your responsibility to check your email and adhere to all timelines throughout the process.

Please note you cannot appeal the following incidents:

  • A final grade.
  • Any academic decision you received more than 5 business days ago.
  • Academic results that were impacted by the lack of an accommodation.
